Permission Forms

In order to participate in weekly activities at GraceUnderground, a signed Permission Form must be on file with the Youth Group Director, Pastor Dana.  Please use the second attachment below.

Please use the Waiver and Permission to Transport Form for the upcoming event.


Here's how to access and use the Permission Forms:

1.  Click on the attachment to open the form.

2.  Print the page.

3.  Fill in the form, complete with your parent's/gaurdian's signature.

4.  Be sure that the form is received by Pastor Dana by the deadline he has set.
